John Lewis Elementary School Attendance Form

Please use this form for all for all attendance matters (absent, tardy, early dismissal, etc.). Please review the DCPS Attendance Expectations. A note must be received within five (5) days after the first day of absence. Students are late after 9:15am, and students are expected to be at school every day, on time, and stay for the duration of the school day.
